THE OBJECTIVE OF 32 DENTAL SOLUTIONS IS TO PROVIDE HIGHEST LEVEL OF DENTAL TREATMENT AT AFFORDABLE COST TO OUR PATIENTS AND WE STRICTLY BELIEVE THAT WE OUR POLICIES SHOULD BE TRANSPARENT BEFORE, DURING AND AFTER TREATMENT. THEREFORE, PLEASE GO THROUGH THE DISCLAIMERS FOR HASTLE FREE TREATMENT.

32 DENTAL SOLUTIONS HAVE RIGHT TO REJECT TO ANY PATIENT FOR THEIR CONSULTATION OR TREATMENT BEFORE/DURING/AFTER THE CONSULATION OR TREATMENT

 

PATIENTS REGISTRATION

  1. Identity – Name, Contact Number, E-Mail ID, DOB (Everything Fields which we ask is significant and is required. 32 Dental Solutions will not reveal your Fields Publically and it will only be used for Communication, Educational and Bills/Reports Delivery only.
  2. Medical Conditions – Patient will have to clearly specify their Medical Status - Any Medical Problems in Past or in Present. 32 Dental Solutions will not be held responsible if any problem occurs due to patient’s negligence of not letting us know about his/her medical Problem because we may need to completely change or Improvise our treatment approach due to different Medical or Systemic Conditions accordingly.
  3. Medication - Patient will have to clearly specify if they are under any sort of Medications – Due to any disease (BP, Sugar etc.) or self-medication - Multivitamins etc. 32 Dental Solutions will not be held responsible if any reaction occurs due to patient’s negligence of not letting us know about his/her medication because we may need to completely change or Improvise our treatment/medication approach accordingly.
  4. Allergies – Patient will have to clearly specify if they are suffering from any sort of allergy in the past or presently. Even though If no known allergy is present - Local Anesthesia, Antibiotics, Analgesics and other medications can still cause allergic reactions causing redness and swelling of tissues, pain, itching, vomiting, and/or anaphylactic shock (severe allergic reaction). In case of severe allergic reaction, he/she may need to rush to the hospital and get the emergency treatment done there. 32 Dental Solutions will not be held responsible for any kind of allergic reactions.
  5. Pregnancy Status (For Females) – Females will have to clearly specify about their Pregnancy Status (Regardless of any number of Days/Week/Months). 32 Dental Solutions will not be held responsible if any Problem occurs due to patient’s negligence of not letting us know about her Pregnancy Status because we may need to completely change or Improvise our treatment/medication approach accordingly.

APPOINTMENT – TERMS & CONDITION

No Refund will be provided to the Patient after Appointment or Consultation Payment. Patient cannot cancel his/her appointment after appointment booking. The online consultation paid amount is for consultation only. The diagnostic cost like x-rays (RVG, OPG & others) are separate and is not a part of consultation amount. The Consultation amount will not be adjusted in any treatment amount. We do not guarantee for any treatment/procedure other than consultation on the first visit until emergency treatment is not required. The appointment for treatment/procedure will be provided to the patient after first consultation visit. The consultation amount is valid till 7 working days or till the treatment is finished of what patient has opted and paid. We can any time reschedule/ Cancel the patient’s appointment according to the availability or last minute emergencies. Patient can’t miss his/her appointment after booking under any circumstances. If the patient cannot come on given his/her appointment time slot, Patient will require to book another appointment slot and Patient will be charged separately for the new appointment booking. Once the patient misses his/her appointment, we will not adjust the booked appointment to another day/time. Patient will choose the Time and Date for the appointment as per his/her comfort, but there could be a waiting time at the reception according to the number of patients. 32 Dental Solutions does not guarantee “No Waiting Time”, but priority will be given to patient who paid & booked online. An appointment “E-Coupon” will be send to the patient through mail and what’s app, which has to be shown by the patient at the reception during their Clinic Visit in their registered phone number. Member cannot cancel his/her appointment after appointment booking. Appointment for consultation can be taken on phone/online/walk in.

APPOINTMENT FOR PROCEDURE BY PAYING TDCA - TREATMENT DATE CONFIRMATION ADVANCE 

Appointment for procedure has to be booked after paying TDCA (Treatment Date Confirmation Advance) – 1000 INR. Appointment when booked after paying TDCA, if missed/rescheduled by the Patient due to any reason will be chargeable as Appointment Missing Charges (AMC)- INR 500/- from the patient accordingly, if 48 hours’ notification as a text request for rescheduling it is not provided either by patient or the clinic. Appointment rescheduling not within 48 hours of appointment will be considered as missed appointment and is chargeable.  AMC (Appointment Missing Charges) will be deducted from TDCA according to the missed appointment, and another TDCA amount has to be given if the patient misses the given appointment 2 times in any manner. At the end of the procedure, the remaining balance of the TDCA amount will be adjusted and not the initial. TDCA will not be adjusted for relative, family member or any other patient other than the patient on whose name TDCA was applicable. It is compulsory to bring this Booklet to get your TDCA refunded.  TDCA will not be refunded in case you do not bring this booklet.

Appointment Booklet – On your First visit of your Consultation, a clinic patient record booklet will be issued to new patient which is part of the Consultation Fee. If any Patient/Member does not bring his/her appointment in any visit post Consultation,  a new Booklet will be issued which is chargeable for Member/Non-member. (Please Bring your Booklet along with you as this is important for Keeping Perfect record, which is beneficial for both, you and the operator). We can any time reject for your Treatment in-spite you are ready to pay for the booklet.

Membership Refund Protocol : We have Discontinued "My Healthy Mouth Membership Program". There is No Membership & Membership Benefits from now on. The Earlier Members whose Membership Tenure is Pending can demand for their Membership refund after settling the amount of membership benefits from the Membership Cost. In cases where Membership Benefits taken by the Patient is more than the Membership cost, No Refund is Possible.

TREATMENT - TERMS & CONDITIONS

Consultation & Diagnosis – The consultation Charges is valid till 7 working days or till the discussed treatment is not finished in single go. If the patients want to take time to think and return back after more than 7 days for their Planned treatment, he/she will require to pay separate consultation charges as the degree of problem may change and there could be a requirement of improvisation of treatment plan altogether.

PPE Kit (Personal Protection Equipments) / Covid-19 Measures Charges  – During the Covid-19 Period, we are using Disposable PPE kits for the Protection of our Patients. The charges of these PPE Kits/Measures are 500/-INR for Each Patients for each appointments. (Note: We always use New Disposable PPE Kits which is opened in fronth of the Patients for Better Transparency & Trust). The Defined Cost is on the basis of both Covid-19 Measures & PPE Equipments, that includes PPE Gears for the Patients (Headcap & Shoe Covers), Doctors (Disposable PPE Surgical Gowns, Headcaps, 3 Ply Masks, Face Shields), Attenders (Headcaps, 3 Ply Masks, Face Shields) & Clinic (Thorough Chemical Disinfection & Fumigation of Operatories before and after each Treatment) and much more. We will not Treat or Consult any Patients without these PPE Kits for their and other Patient's Safety during the Covid-19 Period.

X-Rays & Other Diagnostic Services - The Diagnostic X-Rays - CBCT, RVG, OPG & Soprocare Diagnosis are separate and is charged separately. The OPG & RVG are Digital and will only be mailed to the patients. We do not provide X-Ray film print outs and we only mail them. There are no extra charges for Procedural RVG which we do during the treatment. These RVG are not mailed and purely for in-office use. For 3rd Molar (WISDOM TOOTH) eruption problem, OPG is mandatory. Incase of CBCT, we do the E-Reorting in the form of PDF, which is mailed to the Patient along with DICOM Scan File. While for CBCT (Implant & RCT) we do now charge from the members if they go ahead/Books the Discussed Treatment witin One Month of the Scan. In This Scenaro only the DICOM File is mailed with No Reporting. (For More Details - Please Go Through the Membership Section)

Appointment for Treatment - Appointment for Treatment/Procedure has to be booked after paying TDCA (Treatment Date Confirmation Advance) – INR 1000/-which will be adjusted at the last visit of the treatment and will not be returned once paid after appointment booking. TDCA amount is non-refundable for appointment cancellation at any given point of time.  Do not Cancel your appointment at the Last Minute. At least Inform us 48 hours Prior if there is any change/rescheduling of appointment. As a Rule, if we (32 Dental Solutions) cancels your Appointment at Last Minute, we will Provide You Free or Extend Platinum Membership (Worth INR 2000) to our Patients. Fillings – Sensitivity rarely can occur after filling depending on the extent of decay and quality of material used. Normally sensitivity subsides by its own but can require other treatment approach (RCT/Laser Sensitivity Treatment) for the same, which will be considered and charged separately.  You can still get decay on the filled tooth, if you do not maintain good oral hygiene. If decay occurs, it has to be treated again i.e. Re-Filling or RCT, which will be charged separately. There is no warranty for the fillings.

RCT - Any time in future if pain on biting occurs i.e. infection occurs; Re-RCT/EXTRACTION need to be performed. There is no warranty for the RCT. Remember it is uncertain to determine the life of treated tooth. Although we use new and sterilized files for cleaning and shaping the root canal, very rarely the instrument can fracture in the canal due to mechanical failure. The outcome of the treatment will depend upon many factors, i.e. location of the breakage, curvature of the canal and many more. The end result could be extraction of affected tooth. In such cases the after treatment will be considered as separate treatment with separate cost. Cost of the core build up and crown are separate in respect to RCT. All the procedural X-Rays & temporary filling charges are included with no extra charges.

Crowns - Warranty of the crowns will only be applicable for crowns and not for tooth. Warranty does not cover two things i.e. any damage to the tooth and crown dislodgement. If the damage of Crown is in warranty, the crown will be repeated and you need to bear 20% of the New crown charges that point of time as handling charge. You need to keep the warranty card safely or should know the number of the card. There is no warranty if patient misplaces the crown/bridge.

Implants – CBCT is Compulsory for Implant Planning which is charged separately (Free for Members). The warranty of the implants will be in accordance to the implant system used which will be told initially. The warranty covers replacement by new implant and not to refund. In case it's not practical to replace the implant, the 50% cost of the implant will be refunded after Implant Retrieval. You need to understand that implant can fail due to poor oral hygiene and other systemic diseases. So, proper oral hygiene should be maintained in order to avoid infection or Bone Loss .

Orthodontic Treatment – Although orthodontic treatment provides you correction of misaligned teeth to perfection, but relapse can occur any time, if retainers are not worn properly over a given period of time. There is no warranty, if relapse occurs due to patient's negligence. Patient will need to pay extra if fracture of wire/bands/brackets/etc. occurs due to their negligence. There will be no refund,if the Patient may not continue or doesn’t want to continue the Orthodontic Treatment due to any reason.

Extraction – first 24-48 hours are utmost important post extraction, so that no complication develops. If any complication (DRY SOCKET) occurs due to patient’s negligence of not following up post-operative instructions properly, treatment of it will be necessary and will be regarded as separate treatment and the cost will depend upon the severity of the condition.

Scaling –It does not cause mobility since it does not cause tooth structure loss, but it is due to removal of calculus chunks which holds the teeth. Similarly, sensitivity depends upon the degree of enamel loss prior to the procedure and root exposure after calculus removal. If sensitivity remains after primary care, you need to get Laser sensitivity treatment done which is charged separately (Free for Members).

Bleaching – It will drastically improve the colour of the tooth. How long your results will last is variable and greatly depends on your habits. Bleaching can lead to sensitivity which can require other treatment approach (RCT/Laser Sensitivity Treatment) for the same, which will be considered and charged separately.

Laser Gum Treatment - Since different patient respond differently with lasers, there is no warranty of the laser treatment. You need to understand that laser is a conservative, intermediate approach for variety of procedure like sensitivity reduction, sulcular debridement etc. If you do not respond to laser treatment, you need to get conventional treatments for correction of problems \

Denture – There is no warranty on Denture Fracture due to fall and its rectification will be charged separately. Patients may develop certain sore areas (ulcers) after initial denture use which is normal and will go away by its own after relieving the denture high points. Any Procedure (Alveoloplasty) for preparation of arch to receive denture is a separate treatment and the charges will be told by the doctor in accordance to the amount of alveoloplasty required. Any Implant Prosthetic components for denture attachment is separate and is charged separately.

3 Shape Digital Smile Designing – Digital Smile Designing is an excellent tool for (virtual mock-up) assessment of the looks which you will get after treatment, without undergoing treatment. The final results may vary (superior/inferior) in accordance to the virtual initial mock up result.

Changes in the Treatment Plan - During treatment it may be necessary to change or add procedures because of condition found while working on the teeth that were not discovered during examination. Any changes in the treatment plan is separate and will be charged separately in addition to the Initial Treatment Plan.

 

CONSENT

Before Starting any Consultation or Procedure, the Patient will have to Provide Consent to 32 Dental Solutions that they have gone through each and every Terms & Conditions in detail and they have accepted the terms and condition after fully understanding them. A Complete Consent form will be by Mail/What's App prior to any Treatment. If the Patient Opts for Treatment, it will be understood that the Patient Has Legally accepted all the Terms and Conditions and 32 Dental Solutions cannot be held legally for any patient confusion.

For Minors, if not possible with them, their parents will need to accept the terms and conditions
